Transition from school to adulthood has been a significant priority of major federal and state agencies serving persons with disabilities for many years. Clearly as a priority greater emphasis and attention have been given to youth with special needs as well as "at-risk" youth and the specific challenges they face as they move from school into adulthood. The Individuals With Disabilities Education Improvement Act of 2004 (IDEA 2004) has strengthened the focus on transition outcomes. Each state is required to respond to 20 indicators within a State Performance Plan. Four of these indicators impact transition outcomes: graduation rates, dropout rates, the Transition IEP process, and student outcome survey responses. This intense focus is forcing states to examine how transition services are developed and delivered. This has not occurred in the author's collective professional experiences.

This third edition of Transition IEPs: A Curriculum Guide for Teachers is a practical guide for parents, educators, and practitioners who are involved in the transition planning process and IEP development of students of all disability levels. The narrative chapters provide up-to-date information, functional goals, and easy-to-follow templates in assisting the educator in developing meaningful student outcomes after high school. Student case studies provide a correlation between planning and success.
